Amazon enables 2FA, so what's holding up everyone else?

Amazon is yet the latest in a number of companies that have 2 Factor Authentication available to further secure users' accounts. Very late to the party but at least they have it as an option now. Google, Microsoft and many of the big hitters all support 2FA now yet we still have have holdouts or companies using odd implementations of 2FA (I'm talking about you LinkedIn).

You now have 2FA, passwords managers like Lastpass (with 2FA as well), biometrics and hardware keys like Yubikey (still a bit expensive right now) to secure you accounts. Despite that we still have frequent breaches due to passwords getting stolen or guessed or other security lapses not related to software flaws/glitches.

So my questions are: Are you using 2FA and how extensively? Would you roll it out to your users/clients if you could implement it easily? Just what do...
